Dr. Price is a contracted provider for all major insurance companies as well as Medicare.

Due to the vast number of individual insurance plans out there today, the only way to be 100% sure that Dr. Price is contracted is to contact your insurance company.  The phone number should be listed on the insurance card.
If your insurance requires a referral from your primary care provider, it is the patient’s responsibility to have this completed prior to your appointment.  Our office can assist you in trying to have this completed, but ultimately it is the patient’s responsibility.  If the referral is not available at the time of your appointment, you may be asked to reschedule your appointment.

Please be sure to bring your current insurance information with you to each appointment.  If your insurance has changed, we need this information as soon as possible.  If we do not have your correct insurance information, we cannot file your claim.

Our office will file your insurance claim for you.  Once the insurance claim has been processed we will forward the patient responsibility (deductible, co-insurance, copays, etc.) to you.

All self-pay services not covered by your insurance company and in office cosmetic procedures require payment at the time of service.  Cosmetic procedures performed outside of the office require payment in full 10 days in advance of the procedure.
